1. To finish your registration, click the button in your email.

2. To ensure a smooth visit, please bring:

• Your government-issued photo ID and insurance card
• The bottles or labels of any current medications
• Relevant medical records, if available

3. New Patients:

• Arrive 10-15 minutes early to check in.
• Take all medications as usual unless told otherwise.
• If you recently visited a hospital or ER, bring records and notify the front desk.
• Select a Best Care provider as your PCP on your insurance plan, if required.
• Verify your appointment is with the provider you expect and that they meet your preferences, including language or gender.

Please Note: A 4% fee is added to all card transactions. If you need to cancel/reschedule, contact us at least 24 hours ahead to avoid a $15.00 cancellation or no-show fee.
